  •  We will send out a case management informational packet with fee structure
  • We will schedule a telephone or virtual meeting to assess and evaluate the current situation.
  • We will outline an in-depth treatment plan for 30 to 180 days to meet the client’s needs.
  • We will provide ongoing evaluation weekly or monthly as per the client’s needs.
  • One of our case managers will engage weekly with clients 
  • One of our family case managers will support family members with coaching

Five Stages of Case Management

Engagement

Enrollment, building relationships, creating a structured action plan.

Planning/Goal Setting

What is the CanAm Case management approach?

To navigate wraparound services for  individuals and their families by helping to manage the unmanageability of addiction and/or mental health. Case management gives everyone an opportunity to be treated by a professional. 

  •  Many people choose family coaching services because they cannot dedicate the time necessary for organizing their loved one’s care. 
  • Case Management will help minimize the emotional pressures families and/or individuals experience while building on healthy relationships through the recovery process.
  • Case management is one of our wrap-around services that helps you navigate from the beginning of treatment through your aftercare recovery programs and, in many cases, beyond. We provide professional support and guidance focusing on accountability for the recovering individual as well as the family and friends who support them.
  • We don’t believe there is a “one-size-fits-all” approach to someone’s care plan. Every year, thousands of people enter inpatient treatment centers to recover from substance abuse disorders and/or mental health conditions. Treatment programs provide a safe, structured environment to get emotionally stabilized and start the recovery healing process

We work with other medical specialists, therapists, and many types of addiction and mental health programs to ensure the client is surrounded by professionals, working together for successful and long-term healing and recovery.

  • We help families to navigate the treatment maze in the healthcare industry.  
  • We interview, evaluate and personally tour treatment centers before making referrals.  
  • We collaborate and communicate with clients and professionals during the process of treatment.

Action Steps for Case Management:

  2. Client assessment: gaining information on the client in three different sessions helps to build a relationship with the client.
